Daxxify (daxibotulinumtoxinA-lanm, made by Revance) entered the aesthetic market as a distinct option in the neurotoxin category that dominated injectable aesthetics for decades. Its primary differentiator: duration. Where Botox, Dysport, and Xeomin typically last 3–4 months, Daxxify's FDA approval was based on trial data showing a median duration of approximately 6 months in forehead and glabella lines, with some patients maintaining results for up to 9 months.
That's a meaningful difference — in theory, fewer appointments per year. In practice, the picture is more nuanced.
How Daxxify Works
Like all botulinum toxin products, Daxxify temporarily blocks the neuromuscular junction, preventing the muscle from receiving the signal to contract. The mechanism is the same as Botox or Dysport. The difference is in the formulation.
Conventional botulinum toxin products are formulated with human serum albumin as a stabilizing protein. Daxxify uses a proprietary peptide-based stabilizer instead. Revance's research suggests this peptide excipient affects how the toxin behaves at the neuromuscular junction and may contribute to the extended duration — though the exact mechanism of longer duration isn't fully characterized in published literature.
Clinical Evidence for Duration
The FDA approval for Daxxify (for glabellar lines) was based on the SAKURA trials. Key findings:
•Median response duration: approximately 6 months, versus 3–4 months for conventional toxins
•At 6 months, a meaningful portion of trial participants still showed response
•Some patients experienced duration extending to 9 months
Duration varies by individual — metabolism, injection technique, muscle mass, and patient factors all affect how long any neurotoxin lasts. The SAKURA trials showed a distribution of outcomes, not a universal 6-month result. Some patients will see shorter duration than the median; some will see longer.
What's Different About the Patient Experience
Onset: Daxxify's onset appears comparable to or slightly faster than Botox — visible results in approximately 2–3 days, with peak effect around two weeks.
Result character: Patient and injector reports generally describe Daxxify's effect as similar to Botox in terms of quality and natural appearance when dosed appropriately. Some injectors note a perception of slightly faster or stronger initial effect.
Dosing: Daxxify is not dosed like Botox. The units are not equivalent. Daxxify's unit system is specific to its formulation, and providers should not convert Botox units to Daxxify units with a simple ratio. Overdosing risks exist when providers unfamiliar with Daxxify translate dosing directly from their conventional toxin protocols.
Price: Daxxify typically costs more per treatment than conventional neurotoxins — though the per-year cost may be comparable or favorable if fewer treatments are needed to maintain results.
Who Daxxify May Be Best Suited For
Patients with fast metabolizers: Some patients consistently see their Botox results fade at the 8–10 week mark. For these patients, Daxxify's extended duration may translate directly into fewer treatment gaps and more consistent results.
Patients who find frequent appointments inconvenient: Two to three appointments per year instead of three to four is a real quality-of-life difference for some patients.
Patients with event or lifestyle timing needs: A patient who wants 6+ months of consistent results around a major life event may find Daxxify useful.
Who should be more cautious: First-time neurotoxin patients are typically better served starting with a conventional toxin. If a new patient doesn't like the effect, conventional toxins wear off in 3–4 months. If a new patient doesn't like a Daxxify result, they may be waiting 5–6 months for full reversal. Starting with Daxxify before knowing how you respond to neurotoxins introduces a longer commitment to an uncertain outcome.
Current Availability
Daxxify is not available at every practice — it requires training and relationship with Revance's distribution network. Practices that offer it should be able to explain their experience with the product, including how long they've been using it and how many patients they've treated with it.
As of mid-2026, Daxxify remains approved for glabellar lines (the area between the brows) as its primary indication, with off-label use for forehead and crow's feet common among experienced providers who've incorporated it into their practice. Ask your provider about their on-label vs. off-label use when discussing specific treatment areas.
