Botox has been the dominant neuromodulator in aesthetic medicine since its FDA approval for cosmetic use in 2002. The market has since grown to include Dysport, Xeomin, and Jeuveau — all botulinum toxin type A formulations with broadly similar durations and mechanisms. Daxxify, FDA-approved in September 2022, represents a more substantive departure. It's the first neuromodulator to demonstrate consistent duration beyond four months in clinical trials, and it achieves that through a different formulation approach. Here's what the evidence shows and who benefits from it.
What Daxxify Is
Daxxify (daxibotulinumtoxinA-lanm) is a botulinum toxin type A formulation developed by Revance Therapeutics. It's FDA-cleared for moderate to severe glabellar lines (the vertical lines between the brows) in adults.
The key formulation difference from other neuromodulators is the stabilizing excipient. Traditional botulinum toxin products use human serum albumin as a stabilizing protein. Daxxify instead uses a proprietary synthetic peptide (designated RTP004) that serves the same stabilizing function. This peptide-based formulation is what Revance believes contributes to the longer duration — the mechanism is not fully elucidated in the public literature, but the clinical duration data is consistent.
Daxxify is the same general mechanism as Botox — it cleaves the SNARE protein complex that nerve terminals use to release acetylcholine, temporarily reducing muscle contraction at the injection site. The pharmacological action is not novel; the duration is.

Dosing Differences — Not Equivalent to Botox Units
This is a critical practical point: Daxxify units are not the same as Botox units. You cannot convert your usual Botox dose to Daxxify by applying a straightforward ratio.
The labeled dose for Daxxify in glabellar lines is 40 units (Daxxify units). The typical Botox dose for glabellar lines is 20 units (Botox units). That 2:1 ratio is specific to this indication; it does not mean that Daxxify is half as potent or twice as potent across all areas. The unit systems are independent.

Patients whose primary complaint is treatment frequency. If the logistics of quarterly appointments — time, cost-per-visit, disruption — are the biggest friction point in your current neuromodulator routine, Daxxify's extended duration addresses that specifically.
Patients who respond well to neuromodulators but wear off earlier than typical. Some patients metabolize Botox quickly and see results last only 8 to 10 weeks. If you've consistently had shorter-than-expected duration with other products, Daxxify may provide a more stable result.
Patients treating primarily glabellar lines. The clinical evidence is strongest for this specific indication. Using Daxxify for a full-face neuromodulator treatment — forehead, glabellar, crow's feet — extends beyond the clinical data and requires a provider who is experienced with dosing it in those areas.
Patients for whom the cost per session is manageable. Daxxify is not appropriate as a cost-cutting measure; the per-session cost is higher. The value proposition is convenience and potentially fewer sessions per year, not lower overall spend.
Where to Find Daxxify
Daxxify is newer to the market than Botox, and not every med spa carries it. Revance sells directly to licensed aesthetic providers, so availability is growing but is not yet as ubiquitous as Botox.
When searching for a Daxxify provider, look for practices that specifically list it as a service offering, and ask during consultation how many Daxxify treatments the provider has performed. Given the dosing differences from other neuromodulators, provider experience with this specific product is more relevant than general neuromodulator experience.
What Daxxify Doesn't Change
Daxxify does not alter the fundamental nature of neuromodulator treatment. Results are still temporary. It does not address volume loss, skin quality, or structural changes. It does not treat conditions that Botox doesn't treat. The adverse event profile in clinical trials was broadly similar to other neuromodulators — headache, eyelid ptosis, and injection-site reactions were the most commonly reported events.
If you're satisfied with your current neuromodulator results and the treatment frequency is manageable, there's no clinical reason to switch. If duration and frequency are your pain points, Daxxify is worth a provider conversation.
