Before-and-after photos are the single most influential marketing tool in the aesthetic industry. They're what scrolling patients stop on. They're what drives the "I want what she has" consultation. And they're routinely presented in ways that make results look better than they are — not necessarily through outright fabrication, but through the subtler and far more common practice of photographing the same real result under conditions that maximize the visual difference.
Understanding how photos can mislead doesn't make every before-and-after fraudulent. It makes you a better judge of which ones are showing you what the treatment actually did versus which ones are showing you what the camera did.
Lighting Is the Biggest Variable
A before photo taken under flat, direct overhead lighting — the kind that casts shadows into every line, fold, and pore — makes skin look dramatically worse than it looks in person. An after photo taken under soft, diffused, front-facing lighting — the kind used in professional portrait photography — makes the same skin look dramatically better. The treatment may have done something. But if the lighting changed between photos, you can't tell how much.
What to look for: consistent lighting between before and after. Shadows falling in the same direction, from the same angle, at the same intensity. Professional practices with standardized photo systems use the same lighting rig for every photo, which is one reason those systems exist — not just for documentation, but for credibility. If the before photo looks like it was taken in a bathroom and the after looks like it was taken in a studio, the lighting changed more than the face did.
Angles and Distance
A before photo taken slightly above eye level shortens the jaw, emphasizes the nasolabial folds, and makes the lower face look heavier. An after photo taken at eye level or slightly below lengthens the jaw, smooths the nasolabial area, and makes the face look slimmer. The same face, the same day, can look meaningfully different depending on whether the camera is two inches higher or lower.
Distance matters too. A before photo taken closer to the face emphasizes texture, pores, and fine lines through lens distortion. An after photo taken from slightly farther away smooths those same features optically. This isn't editing — it's physics. But it produces a comparison that overstates the treatment's effect.
What to look for: the same angle and the same apparent distance in both photos. The relationship between the ears, jawline, and hairline should be consistent. If the before is a three-quarter view and the after is straight-on, the comparison isn't valid.
Makeup and Skin Preparation
A before photo taken on bare, uncleansed skin shows every imperfection at full visibility. An after photo taken with even minimal primer, moisturizer, or tinted sunscreen smooths the skin surface and diffuses light differently. The treatment may have improved the skin. The product on the skin is also contributing to the visual difference.
This isn't necessarily deceptive — an after photo taken at a follow-up appointment may reflect the patient's normal daily appearance, which includes skincare products. But it's a variable that affects the comparison, and a practice that controls for it by photographing bare skin in both photos is producing a more honest comparison than one that doesn't.
Timing
An after photo taken at peak effect — immediately after a filler treatment when the product is freshly placed and the tissue is slightly swollen — looks different from an after photo taken at six months, when some of the product has metabolized and the tissue has settled. An after Botox photo taken at two weeks (peak effect) looks different from one taken at four months (waning).
The timing of the after photo matters because it determines whether you're seeing the best-case moment or the sustained result. A practice that shows you two-week Botox results is showing you something real — but it's the ceiling, not the baseline. Ask when the after photo was taken, and whether the practice has photos at three or six months to show you what the sustained result looks like.
Editing and Filters
Outright editing — smoothing skin, adjusting color, reshaping features in post-production — is less common at legitimate practices than the subtler variables above, because it's also more obviously deceptive if caught. But it exists, and social media has normalized it to the point where some providers don't register it as problematic.
Signs of editing include unnaturally smooth skin texture (real skin has pores), blurred edges around facial contours, and inconsistent sharpness between the face and the background. These are the same tells that indicate filtered selfies, and the same critical eye applies.
What Good Documentation Looks Like
A practice that takes its before-and-after documentation seriously will use standardized conditions: the same camera, the same lighting rig, the same background, the same distance, the same angles (typically front, three-quarter, and profile), bare skin, and labeled timing. The question is whether the visual difference you're seeing reflects the treatment's effect or the photography's conditions. Providers who standardize their documentation are doing you a favor, even when the results look less dramatic — because what they're showing you is closer to what you'll actually experience.
