State medical board license lookup

Every state maintains a free, public database where you can verify any medical provider's license status, check for disciplinary actions, and confirm credentials. Select your state below.

State license lookup data is being compiled and verified. All 50 states will be added shortly.

In the meantime, search for “[your state] medical board license verification” to find your state's free lookup tool.

What to look for

Active status

The license should show “Active” or “Current.” Any status like “Expired,” “Suspended,” or “Restricted” means the provider may not be legally authorized to practice.

Disciplinary history

Check for any formal disciplinary actions, consent orders, or restrictions. A clean record is expected; multiple actions are a warning sign worth investigating further.

License type

Confirm the provider holds a license type appropriate for the procedure: MD, DO, PA, NP, or RN. Different license types have different scopes of practice and supervision requirements.

Board certification (separate from state license)

Board certification indicates a provider completed an accredited residency and passed specialty examinations. Verify board certification through the American Board of Medical Specialties:
